Terms of Service

  • By booking an appointment, you agree to the following:

    1. I give permission to receive myofascial release therapy from Live Once Live Well Myofascial Release.

    2. I understand that the services provided by Live Once Live Well Myofascial Release are not a replacement for medical care, should not be construed as a substitute for medical examination, diagnosis, or treatment, that no medical diagnosis will be made, and that I should see a medical provider or primary physician for any medical issues I may have.

    3. I agree to inform my therapist of any preexisting conditions, current or new injuries, limitations, specific sensitivities, any changes in my health status, or any concerns I may have.

    4. I understand I can ask the therapist to adjust the manipulation, draping, pressure, heat, or environment at any time or, ask the therapist to end the service at any time. If the therapist is unable to relieve my discomfort or pain or uncomfortableness after I request an adjustment, I will inform the therapist I would like to end the service immediately.

    (By booking your appointment, you agree to the following statements)

    I acknowledge that I have received a physical examination and have consulted with my physician before participating in any massage, skin care, stretch service, or stretching regimen.

    I understand that I can end the service at any time if I experience discomfort or pain or become uncomfortable for any reason during a service.

    I understand that the therapist may refuse or discontinue my service, at their sole discretion, if the therapist determines such service may be unsafe or cause me discomfort or if I engage in any inappropriate conduct as determined by the therapist or Live Once Live Well.

    I understand the risks associated with myofascial release therapy include, but are not limited to:

    •       Superficial bruising;
    •       Short-term muscle soreness;  
    •       Exacerbation of undiscovered injury; and/or
    •       An emotional response (e.g., triggering of past or forgotten trauma responses) occasionally occurs and emerges in the form of crying, aggravation, or another emotion responding to the healing crisis. If this happens, it is okay to express your feelings in our safe, nonjudgmental environment or you may request privacy and end the session. You are in control.

    I have been given a chance to ask questions about the myofascial release therapy session and my questions have been answered.

    I hereby waive all claims, assume all liability, and release, hold harmless, and indemnify Live Once Live Well (including their agents and employees) from liability for any injury, claim, cause of action, suit, demand, and damages (including, without limitation, personal, bodily, or mental injury, property damage, economic loss, consequential damages, and punitive damages), arising from or related to their myofascial release therapy services.

    By booking my appointment, I understand, agree, and hereby voluntarily accept all risk and responsibility associated with the services provided and use of the Live Once Live Well facilities located at 2901 Park Avenue, Suite A6, Soquel, CA 95073 or at another location Live Once Live Well services are provided.

  • Live Once Live Well Myofascial Release values your health and the health and well-being of others. Please read the following, COVID-19 Liability Waiver and Assumption of Risk, prior to booking. Thank you!

    I understand that COVID-19 still exists. I further understand that I am the decision-maker for my health care. To the best of their ability, my practitioner will provide me with information to assist me in making informed choices.

    I am aware of the highly contagious nature of COVID-19 which may result in illness, personal or psychological injury, pain, suffering temporary or permanent disability, death, property damage, and/or financial loss. I acknowledge that these risks may result from or be compounded by the actions, omissions, or negligence of Live Once Live Well’s employees or independent contractors, including negligent emergency response or rescue operations of Live Once Live Well.

    I acknowledge that Live Once Live Well cannot guarantee that I will not become infected with COVID-19. I understand that Live Once Live Well has implemented preventative measures and intensified sanitation protocols intended to reduce the spread of COVID-19. However, because this service involves close physical proximity over an extended period of time in a closed space, there may be an elevated risk of disease transmission, including COVID-19.

    I acknowledge that I am voluntarily accessing the Live Once Live Well’s premises and must comply with all of Live Once Live Well’s procedures to reduce the spread of COVID-19 while attending my appointment. I attest that within the past fourteen (14) days:

    ·      I have not been diagnosed with COVID-19;

    ·      I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell; and

    ·      I have not been exposed to someone with a suspected and/or confirmed case of COVID-19.

    I hereby expressly waive and release any and all claims, now known or later known, against Live Once Live Well (including its employees and independent contractors) on account of personal or psychological injury, illness, pain, suffering, temporary or permanent disability, death, property damage, or financial loss arising out of or attributable to being on their premises, whether arising out of the ordinary negligence of Live Once Live Well or any Releasees or otherwise. I covenant not to make or bring any such claim against Live Once Live Well or any other Releasee and forever release and discharge Live Once Live Well and all other Releasees from liability under such claims.

    By booking your appointment you have read and agree to Live Once Live Well’s COVID-19 Informed Consent.

  • I may cancel or reschedule online or by contacting Live Once Live Well up to 48 hours prior to my appointment to avoid being charged the full amount of my session, or session deduction from a package. Appointment reminders are sent so that I’ll have plenty of time to make adjustments to my appointment. I’ll contact the therapist if I don’t receive a confirmation or reminder notification. (Check email or text messages for reminders).

    I’ll be considered a no-show if I don’t contact Live Once Live Well about my absence or don’t show up to my appointment. As a result, I will be charged the full amount, or session deduction of a package, of my scheduled appointment.

    Any late arrivals will receive the remaining time and will be charged for the full duration of the originally scheduled appointment.

    I should expect to be welcomed into a professional and therapeutic environment. All sessions are to be strictly for therapeutic purposes, any sexual advances, conversations, innuendos, or comments will not be tolerated and will result in immediate termination of the appointment at the discretion of the therapist. You will be charged the full-service fee regardless of the length of your session. Depending on the behavior exhibited we may also file a report with the local authorities if necessary.

    I understand that these terms may change without notice and Live Once Live Well will do its best to inform me of such changes, yet I am ultimately responsible for making sure I have a clear understanding of these terms.

  • Package Policy:

    It's recommended to book all 6 (six) sessions upon purchase to reserve your times before someone else.

    This does not guarantee, "fixed-in-six," and session results may vary.

    Packages are non-refundable.

    Packages are non-transferable.

    Packages expire 60 (sixty) days after the first use of the package.
